Walk any plant floor and ask about the annual safety training. You'll hear the same thing: a video of a factory that isn't theirs, equipment they've never touched, a quiz they've memorized. Completion rates look great. Then the incident report reads the same as last year's — because most incidents aren't knowledge failures. They're human-factors failures: people who knew the rule and, in the moment that mattered, didn't apply it.
The fix has been known for decades — practice the decision, on your own equipment, before it's real. Airlines do it. Mine rescue teams do it. But building that kind of training for one facility used to cost $30,000–50,000 and take months. So almost nobody got it.
