Lockout/tagout is one of the most frequently cited standards in American industry, year after year. It's also one of the most thoroughly trained. Nearly every plant runs LOTO training. Nearly every worker has sat through it.

So why do people still get hurt?

Not “do I know what lockout is,” but “does this machine, in this state, require lockout right now?” That gap is the entire problem, and generic training cannot close it.

What generic LOTO training gets wrong

Open almost any off-the-shelf course and you'll see a clean, well-lit, entirely fictional machine. One energy source. Clearly labeled. The correct action obvious.

Your floor isn't like that. Your machine has multiple energy sources — electrical, pneumatic, hydraulic, stored mechanical, residual. Your disconnect is behind a guard, or above head height, or shares a panel with the line next to it. Your procedure has a step that isn't in the manual because of something that happened in 2019.

A worker who aced the generic module can still stand in front of your equipment and not know whether to lock out.

The other failure: the training doesn't distinguish between people. A twenty-year maintenance tech and a contractor on his second day get the identical module. One is bored. The other is lost. Neither is being trained.

The moment that actually matters

The hardest LOTO question isn't procedural. It's the decision point: is this servicing and maintenance, or a minor servicing activity during normal production? Do I lock out, or does an alternative measure apply?

Workers get honestly confused here, because the answer depends on the task, the machine, and the circumstance. Reading the written procedure doesn't resolve it — people can only interpret so much from a document.

That decision — under time pressure, on a machine that's still warm, with a supervisor waiting — is what training has to rehearse. Not the definition of lockout. The judgment.
